Disability Documentation Requirements for Service Eligibility and Determination
Based on Psychiatric Disability

The following information must be provided, on official letterhead, by a licensed professional qualified to diagnose and treat the student's disability. Review of the documentation will enable SSD staff to determine student eligibility for services and facilitate the appropriate support and reasonable accommodation of the student's disability-related needs. Please have such documentation mailed or faxed to B. • Short Term Prognosis
• How the student's disability may affect him or her at Binghamton University
• Medications the student is currently taking and how they affect him or her
• Recommended reasonable accommodations
• Type of therapy the student is currently receiving
• Name, address, phone number and credentials of present therapist
• Permission to share this information with the appropriate professionals in the University Counseling Center
